To ensure we can accurately prepare your Testing Agreement and proceed without delays, please read and follow the instructions below:
Multiple products can be submitted in one form – If you have more than one sample or formula, you may list them together in this form. Please clearly differentiate each sample in the sample information section.
Be specific and complete – Provide detailed information about each sample, including its name, appearance, known components (if any), and testing objectives. The more details you provide, the better we can plan the appropriate testing process.
Project background – If this test is part of a product development, competitor analysis, regulatory compliance, or quality control project, please mention it clearly.
Preferred turnaround time – We will do our best to meet your desired timeline. Actual lead time depends on the complexity of the tests and the type of sample.
Confidentiality – Please ensure a Non-Disclosure Agreement (NDA) has been signed prior to submitting this form.

General Terms and Conditions of Testing Services
Agreement and Signature
This agreement becomes effective once signed by both parties. Each party retains a signed copy. The client is responsible for providing accurate and complete information. Any changes during the testing process must be submitted in writing and approved by both parties. The service provider may adjust the testing plan, fees, and timeline accordingly.
Scope of Work
The service provider will perform analytical testing and reporting based on samples provided by the client and testing items listed in the application form. Reports are intended to provide scientific insights for research, quality assessment, regulatory compliance, and product understanding. No guarantee is made regarding the replication of any product.
Fees and Payment
Fees are based on the official quotation. Testing will commence after full payment. Delays in sample delivery, incomplete information, or late payment may result in timeline extensions.
Special Requirements
Any client-specific technical requirements must be submitted in writing and signed by authorized representatives of both parties. These documents form an official appendix to the agreement.
Confidentiality
The service provider will maintain confidentiality of client information and test results. No information will be shared with third parties without written consent, except as required by law or regulatory authorities.
Use of Reports
Test reports are intended solely for internal research, quality assessment, and regulatory compliance purposes. The client must not alter, forge, or misuse the reports for unlawful purposes.
Client Responsibilities
The client is responsible for the accuracy and authenticity of samples and information. Known or potential hazards (e.g., toxicity, flammability, radioactivity) must be disclosed. The service provider is not liable for consequences resulting from undisclosed hazards.
Sample Retention
Samples will be retained for 30 days after report issuance. After this period, the service provider may dispose of the samples at its discretion.
Limitations of Analysis
Testing provides technical insights but does not guarantee commercial outcomes, replication of other products, or regulatory approval. Separate agreements may be established for additional technical development.
Cancellation and Termination
If the client cancels after payment, a written request must be submitted. Approved cancellations incur a 30% fee. If not approved, no refund is granted.
Dispute Resolution
Parties will attempt amicable resolution first. If unresolved, disputes may be referred to independent arbitration (e.g., ACICA).
Legal Validity
This agreement is legally binding upon electronic signature via a recognised platform (DocuSign, Adobe Sign, or mutually accepted method). Each party receives one digital copy with equal legal effect. Physical copies are not required unless requested.
